The Poland international scored 30 league goals for the Bundesliga champions last season but is said to be frustrated with life at the Allianz Arena.
Lewandowski has recently hit out at his Roten team-mates for not doing enough to allow him to become the Bundesliga's top scorer, an accolade Borussia Dortmund's Pierre-Emerick Aubameyang claimed with 31 goals last term.
Blues boss Antonio Conte is ready to take advantage of Lewandowski's situation and sign him as the focal point to his attack in place of the departing Diego Costa.

ARSENAL TO OFFER RAMSEY NEW DEAL
Arsenal are ready to open contract talks with midfielder Aaron Ramsey, report the Telegraph.
Alexis Sanchez, Mesut Ozil and Alex Oxlade-Chamberlain are all uncertain about their future at the Emirates, leading Arsene Wenger to attempt to tie his other key assets to the club.

MOURINHO DECIDES AGAINST RONALDO
Jose Mourinho has dismissed rumours linking Cristiano Ronaldo with a return to Manchester United this summer and is instead focused on completing a deal for his Real Madrid team-mate Alvaro Morata.
The United boss is keen to reunite with the Spain international, according to the Express , having handed the 24-year-old his Real debut back in 2010 while in charge of Los Blancos.
United will have to fork out around £65 million for Morata, though the fee is significantly short of the £175m which they would have to pay for Ronaldo.
